It is estimated that the number of drug doses delivered each year by pressurised metered dose inhalers (pMDIs) is in excess of 50 billion; the vast majority are delivered for the treatment of asthma. With so many doses to deliver, there is a need for manufacturers supplying the inhalers to manufacture within strict regulatory guidelines as well as at high volumes.
The technology within a pMDI is surprisingly complex, and individual drug formulations are matched to sophisticated mechanics and materials so that the patient can receive an accurate dose of medication reliably. They are becoming even more complex as, driven by the need to improve both compliance and safety, manufacturers seek to add functionality such as dose-counting mechanisms that show the user how many doses remain in the pack.
The bulk of the inhaler market is divided between pMDIs and the relative newcomers, dry powder inhalers (DPIs) such as GlaxoSmithKline's (GSK) Seretide® DiskusTM. Since their inception, DPIs too have come under the scrutiny of designers keen to improve their functionality and, as a result, inhaler design has become increasingly sophisticated, with ergonomics and dose-counting being routinely incorporated.
